CHECKLIST FOR CLERGY
Working with a Caregiver
Determine
the individual’s needs.
If there are any indications of same
gender desires and actions proceed to the
next step.
Decide
on clergy’s role.
Decide on the level of involvement clergy
will have. With the individual’s permission clergy may involve others such
as a capable family member, a member of the congregation or family friend
to become a caregiver. Monitor the caregivers progress by using the
following checklist. If the clergy is going to be the only caregiver the
checklist can be adapted for personal use.
Meet
with the caregiver.

Teach the caregiver to help the person to
stop same homosexual behavior by using this Guidebook.

Ask the caregiver to read this guidebook
and follow the steps.

Establish a regular meeting time for
clergy and the assigned caregiver.
|
Inform the individual with same gender desires and actions about the assigned
caregiver.
Tell the person that the caregiver will be
assisting clergy. The clergy’s personal involvement will be on an as
needed basis.
Follow-up with the assigned caregiver by reviewing the following steps:
Step
1: Understanding same gender attraction
 | Has the destructive nature of homosexuality been taught? |
 | Has the Understanding Same Gender Attraction handout been given? |
 | What is the level of involvement the individual has with same gender desires and actions? |
 | What is the level of responsibility, spirituality and desire for change? |
 | Are there any associated concerns like mental illness, that may require professional help? |
Step 2: Develop a personal growth plan
 | Has the individual completed the
Consequence Worksheet and reviewed it with the caregiver? |
 | Has the individual completed the Stop
Same Gender Desires and Actions Worksheet and reviewed it with the
caregiver? Eliminated access to all pornography? Practiced eye and
thought control? |
 | Has the
sacred purpose for sexual intimacy been taught? |
 | Has the individual studied selected
materials from Information About Same Gender Attraction table? |
 | Has the individual met with clergy to complete the steps of repentance and
forgiveness? |
 | Has the individual’s family been educated
and strengthened? |
 | Has the individual completed the
Controlling Triggers & Temptations Worksheet and reviewed it with the
caregiver? |
 | Has involvement in LDS Family Services and
other Community Support groups been evaluated? |
Step 3:
Provide on-going help to strengthen growth
 | Has the individual completed and reviewed
with the caregiver another Stop Same Gender Desires and Actions Worksheet? |
 | Has the individual completed and reviewed
with the caregiver another Controlling Triggers &
Temptations Worksheet? |
 | Has the individual continued to study materials from Information About Same Gender Attraction table and
attended support groups? |
Step 4: Referring
for Assistance
 | Has the individual been encouraged by the caregiver to make progress? |
 | Have the various referral options been considered and discussed with the individual and the individual’s family? |
 | Has the individual followed through with the selected referral?

 | Return to Step 3: Provide on-going help to strengthen growth. |
